In the event of the death of a loved one, you can submit a claim by sending an email to admin@chengetadiaspora.com with the following documents attached:
1. Claim Form - A fully completed Claim Form signed by the Policyholder/Beneficiary.
2. Proof of Death – A certified copy of the Death Certificate.
3. A certified copy of the DHA1663 – Notice of Death Form.
4. A certified copy of the Main Member’s ID.
5. A certified copy of the Beneficiary’s ID.
6. Should the cause of death be an accidental death, we require:
• A police report to be completed by the investigating officer at the Police Station where the incident was reported; and
• An investigation report if the cause of death is under investigation to be completed by the investigating officer at the Police Station where the incident was reported.
